Jason Price at Health Based Healthcare uses your information to respond personally to your inquiry, discuss available coverage options, and arrange a consultation. Submitting a form does not enroll you in insurance or require a purchase. Permission for personal follow-up by phone or email does not authorize automated marketing calls or texts.
Who receives your information
Your inquiry is directed to Jason Price. We do not sell these leads or distribute them to other agents for their marketing. Service providers that support form delivery, record storage, email, or appointment booking may process information to provide those services.
Platforms and appointment booking
Meta processes Facebook and Instagram form submissions under its privacy policy. Wix hosts our website. If you choose to book through Calendly, Calendly processes the booking details you provide under its privacy policy. Those services have their own privacy practices.
Retention and your choices
We retain inquiry information as needed to respond to your request, provide services you request, and meet applicable recordkeeping obligations. You may ask us to stop contacting you or request access, correction, or deletion by emailing healthbasedhealthcare@gmail.com or calling 954-547-8342. We may need to verify your identity before fulfilling a request. Applicable legal recordkeeping requirements may limit deletion. Do not send sensitive documents with your request.
